determination 2015-5483 (Rev 22)-see attached; 2) provide an updated statement of work

(SOW)- see attached; 3) provide 2 updated frequency of service (FOS) charts- see attached;

4) provide answers to vendor questions- see attached, and 5) update the final proposal due
